Report a safety hazard, incident or near miss
The University is committed to providing and maintaining a safe campus.
It is the responsibility of all staff, students, contractors, volunteers and visitors to report any safety related hazard, incident or near miss, no matter how minor they are.
Safety hazards - Safety hazards are risks that are proactively identified in the workplace and if neglected that have the potential to cause injury, harm, illness, or property damage.
Incidents - Safety related incidents in the workplace are reflective of an event occurring and which has resulted in sustaining an injury or illness or has led to property damage.
Near misses - Near miss incidents are reflective of an event occurring that had the potential to cause injury, harm, illness, or property damage.
What do I need to do?
If you’re injured seek first aid or medical attention as required. Report any hazard, incident or near miss on campus to a UWA staff member as soon as possible. This could be your lecturer, tutor, immediate manager/supervisor or UWA host. For UWA staff or contractors, the report must be made within the same shift.
You must also promptly lodge a report into our UWA incident reporting system – Camms.
What can I expect?
Once your report is submitted, you’ll receive an automated email confirmation.
Your assigned UWA staff member will also receive an automated email notifying them of your submitted report and they are responsible for reviewing, actioning and closing the incident report.

What other services will I need?
When a hazard, incident or near miss relates to UWA property or infrastructure, this must also be reported to the Campus Management Helpdesk at [email protected] or 6488 2025. If it occurs after hours, please contact Security at [email protected] or call 6488 3020.
